Puckered Panniculus Sign

 

  • Plain film abdomen finding
  • On abdominal radiographs, the fatty panniculus, if present, has two configurations
  • Smooth, uniformly convex bow which is the normal appearance (yellow arrows below)
  • Or, a bow with a central indentation (broken bow) (blue arrows below)
  • Latter is a useful radiographic sign of previous lower abdominal surgery
  • Central indentation is created by tissue tethering around the laparotomy scar
